I went back and checked a few places where it happened- nearly always with the change in rooms. The screen-stuttering thing I mentioned in the thread I posted earlier happened with DirectDraw on, and right at points where the autosave kicked in. If you shut down the lights after restoring the water, then you can get all three. Regardless of which approach you use first, you can still solve that puzzle two ways minimum, and get both achieves. I did find one circumstance where you could get more than one of them in a given playthrough for the same puzzle- it was the ones with the Trance Den entry. Speaking of achievements, I've been having a pile of fun with those. Good luck with the achievements!
